How we use your information

  • We collect and hold data about you for the purpose of providing safe and effective healthcare
  • Your information may be shared with our partner organisations to audit services and help provide you with better care
  • Information sharing is subject to strict agreements on how it is used
  • We will only share your information outside of our partner organisations with your consent*
  • If you are happy with how we use your information you do not need to do anything
  • If you do not want your information to be used for any purpose beyond providing your care please let us know so we can code your record appropriately
  • You can object to sharing information with other health care providers but if this limits your treatment options we will tell you
  • Our guiding principle is that we are holding your information in the strictest confidence

*Unless the health & safety of others is at risk, the law requires it or it is required to carry out a statutory function

We offer our patients the choice of having a Summary Care Record (SCR). You may already have an SCR.

What is the NHS Summary Care Record?

The Summary Care Record contains basic information about:

  • any allergies you may have,
  • unexpected reactions to medications,
  • and any prescriptions you have recently received.

The intention is to help clinicians in A & E Departments and ‘Out of Hours’ health services to give you safe, timely and effective treatment. Clinicians will only be allowed to access your record if they are authorised to do so. You will be asked if healthcare staff can look at your SCR every time they need to, unless it is an emergency, for instance if you are unconscious. You can refuse if you think access is unnecessary.


You can chose to have other useful information added to your SCR and have an Enhanced Summary Care Record.


This will include:

  • Your illnesses and health problems
  • Operations and vaccinations you have had
  • How you would like to be treated (where you would prefer to receive care i.e. in your own home)
  • What support you might need
  • Who should be contacted for more information about you

If you would like to add other useful information to your SCR please discuss with your GP.
