Social prescribing helps people to overcome both physical and mental health issues by making connections with the people they need to support them.
Minerva PCN practices provide a Social Prescribing Service for patients in our practices.
Our Social Prescriber Link Workers will listen and help you identify your goals, connecting you with individual and community solutions to help with non-medical issues such as loneliness, money worries, housing and leisure activities.
What is social prescribing?
Social prescribing is an NHS model that helps connect you with the community to manage your health and wellbeing. There are lots of people, activities, groups and services in the community that can help meet your needs. For example, volunteering roles, cooking classes, sports and more.
Getting a social prescribing link worker

The Social Prescribing Service is designed to support patients with a wide range of social, emotional and practical needs. Recognising that health is determined by social, economic and environmental factors. Social prescribing seeks to address patient’s needs in a broad and holistic way. Some of the patients who access the service are worried about their financial situation or housing, feeling very lonely and isolated or stuck and unhappy.
Rachel Dawson is the Social Prescribers for the Minerva PCN practices and she works with patients over a short period of time to explore what most matters to them and through guided conversations, identifies solutions which will improve their general wellbeing. This often involves linking patients with services and activities in the community.
Patients 18 and over can access the service by booking a telephone triage appointment.
